چکیده

W nephrologists wait for the ideal, non invasive, inexpensive, precise, and reproducible tool to evaluate extracellular volume (ECV), they need to exert their clinical acumen in the quest of that holy grail, dry weight (DW). Estimation of DW using a clinical approach based on blood pressure (BP) and ECV is feasible and reliable as shown by successful experiences in various dialysis modes over more than three decades. But a need still exists to resolve difficulties associated with accurate assessment of BP (methods and circumstances of measurement, and the confounding effects of antihypertensive drugs) and ECV (evaluation of weight changes unrelated to ECV, lack of specificity and sensitivity of clinical symptoms, lag time, confusion in terminology). An essential point in clinical assessment of DW is that a normal BP is at the same time the target and the crucial index of DW achievement. For this reason, a trialand-error “probe” process has to be used at intervals to make sure that the dry weight target point is correctly estimated. The various “non clinical” methods proposed for dry weight assessment increase the complexity and the cost of hemodialysis. They are, in the present state of things, more clinical research than practice tools. They do not replace clinical judgment.

Application of Bioimpedance Spectroscopy in Asian Dialysis Patients (ABISAD): serial follow-up and dry weight evaluation

BACKGROUND Maintenance of the optimal fluid status in hemodialysis patients is still a challenging task in daily clinical practice. A bioelectric impedance technique has been applied for evaluation of hydration status in the dialysis population in recent years, but serial observations of its role in clinical dry weight determination are lacking. In this study, serial follow-up data of a body co...

Biochemical Markers of Acute and Chronic Ethanol Use: Forensic and Clinical Applications

Background: The analysis of ethanol in bio-samples is an important method for determination of acute alcohol use/abuse both in clinical and forensic toxicology. In forensic cases, it is known that micro-organisms involved in the postmortem putrefaction process can produce alcohol and when the body has been traumatized. In clinical setting, post-ingestion time has a critical role for determinati...

Determination of absorbed ratio in I-131 dosimetry using MCNP-4A [Persian]

Determination of the total body absorbed dose consists of two parts: Beta radiation absorbed dose and gamma radiation absorbed dose. The first part is generally determined by clinical data, while the second part has a parameter called absorbed ratio and is determined by computational methods. The Monte-Carlo computational methods have been shown to be the most suita...

Numerical Modelling of Dynamic Compaction in Dry Sandy Soils for Determination of Effective Print Spacing

Dynamic compaction (DC) is a widely used soil improvement method in dry and/or saturated soils. The design of DC treatment is usually carried out based on the past experiences and empirical relations. In order to clarify the ambiguities in DC processes, numerical modeling of DC in dry granular soils has been considered using a dynamic finite element code. Using the developed model, two design c...

DETERMINATION OF THE OPTIMUM HEAT STRESS INDEX AT A FOUNDRY

This paper presents a case study to determine the efficiency of the currently used heat stress indices in correlation with wet bulb globe temperature (WBGT) as a permissible index at a foundry plant and to evaluate the extent of the physiological responses such as heart rate and body temperature in correlation with heat stress indices. The field consisted of a foundry plant where a shift w...
